MCNP Class Registration for Non-US Citizens

(including Lawful Permanent US Residents)

To register for classes we need the following information submitted via an email message:

The class you wish to enroll in

Your full name (including perferred title, Mr. Mrs. Dr. Ms. Miss)

Your employers name

Employers street address, city/state, country and zip or postal code

Your phone number at work

Your country of citizenship

Your passport number and full expiration date (month/day/year)

Your city and country of birth

Your date of birth (month/day/year)

Please also attached to the email a copy (either PDF or MS Word) of your resume or CV
